1. Due to current strict control of COVID-19, it is decided that all presenters please pre-record your slide show. Please send in your pre-recorded slide show video file to tech-committee@pof2021.org.

2. Please send in the video file on or before Nov. 19, 2021.

3. We apologize for the short-notice. As always, safety is of top concern.

Registration is open to the public.

This same Form is to be used for both Individual and Group Registration, defined as more than two registrants from the same institution or company are registered together.
